RELEASE/WAIVER:

I hereby agree to indemnify and hold harmless Scented Wicks Candle Bar and its employees against all claims for personal injuries or damages of any kind arising from participation in any Candle Making workshop, classes, and their resulting products. *

PHOTO CONSENT:

By your signature, you agree that may use the below-named participant’s photograph in the routine promotion of its activities.

SUBSEQUENT VISITS:

This waiver shall be kept on file and adhered to for both initial and subsequent visits to the workshop by the participant. You may revoke this waiver at any time by sending a statement via certified mail to:  3109 N. Main Street STE 102 Hope Mills, North Carolina

I fully understand and acknowledge that the workshops offered at Scented Wicks Candle Bar may involve inherent risks, dangers, and hazards that may result in injury or illness. I hereby voluntarily agree to release, waive, discharge, hold harmless, defend, and Scented Wicks Bar, its managers, members, and employees from all claims, actions, or losses for bodily injury, property damage, wrongful death, or otherwise, which may arise from my participation in any workshops or classes, resulting products from workshops or classes, or from use of equipment provided by Scented Wicks Candle Bar.  I accept full liability for creations made
by the participant in the workshop.
